Medical paper for school admission in ISCK from Bayan

Dear Readers,
I registered my kid to join in ICSK and the school admin gave us a file containing some forms including Medical form to be filled by the doctor. They also advised me to go Bayan to get Medical certificate.

Could any of you went through this process and got Medical certificate from Bayan.

Please help me where to go and how to get it. I need to plan it in advance to get short time permission from the company.

Its open from 7 am.
Take the documents mentioned in your paper.
Take your child with your for few tests.
Approx time - 15 Mins.

Pls take all pepers two copies Xerox which includes paasport , birth certificate, civil id

Thank you MD & Desperate. I reached around 7:40 and i am the first person to go. Fininshed all the process in 20 min and got the Health file for my kid.

just visit your clinic area on civi id and meet the child doctor or any doctor this will take 15/20 minutes only to get a file made this is so fast and simple you must take the child also they will do vaccination too.
