CIVIL ID ADDRESS CHANGING PROCEEDURE- AS OF 28 APRIL 2010

1.Rental contract copy(keep orginal in hand if u r the rental owner like to change own adds others need copy only)
2.Civil id application form duly filled and signed by u(whom like to change adds)
3.Address changing form duly filled and signed by U,if u are not the owner it should be signed by the orginal rental owner(form will get from typing centers or from PACI only)
4.copy of passport including iquama stamped page.copy of civil id of you and civil id copy of rental owner
5.For changing photo keep one photo with u (6X4 blue background)also keep orginal passport for photo changing nowdays they are asking orginal passport
6.For babies need blood group certificate duly filled and signed by any MOH doctor(form wil get from typing centrs)
7.Each person must have a new envelop duly filled(3 persons in a family need 3 covers duly filled)
8.Go in person at morning time 7.30 am PACI office south surra,take number from outside counter by showing ur civil id(for babies sponsors only)
