I do it for my A/c last year, No any such big issue
1. Fill the form (Bank will send it to you) - No need to download from anywhere except direct bank website.
2. Attach the copy of Passport, Pan card, Civil ID (kuwait), Iqama copy, Salary proof (itmaad taukiya like) - No problem, if its in arabic, must sign all papers
3. Send to your known one - Submit the hard copy there & take receiving with the stamp (Must).
4. Must attach one letter to the General manager of that bank (your branch only)
5. Send scan copy PDF via email too. That email address is different from where the mail comes to you. - As I remember.
It's not so urgent issue for general NRE a/c, but compulsory for whom who have NRE a/c with different currency like $, yen, Can$, Aus$, B. Pound
But Submit it must within a specific time (if mention in email)
