Recently i completed my baby Visit Visa to Dependent Visa Conversion.
Followed the same procedure in the below link,
http://www.indiansinkuwait.com/IIKForum/threadView.aspx?id=75889
Process in Visa Office is same as mentioned with few changes.
No need of marriage certificate attestation. Only Birth certificate attestation and translation is required.
Some changes in Medical process.
There is no need of Medical Application form. Once we took the printout of Medical approval paper in the backside of Visa Office and got approval in the counter. We have to go to computer section(counter 22 i think) to request civil ID number which is backside of 1 -10 counters.
They asked to come back after two days to get Civil ID number.
In the mean time i went for medical(No need of baby but i took along with me as i don't aware) with Original passport of baby, Medical Approval paper, Visa Copy and Father civil Id copy.
Went to Family Counter (7). They will check the documents and put 10KD stamp in Passport. They will keep those documents and sent to Room No. 8.
Provide the passport to them , they gave one form with Polio details and BCG/Xray detail which we need to get stamp and sign from doctors.
For BCG/Xray approval went to TB unit Room 16. Got stamp after checking original Vaccination card and they sent to get doctors sign.
Once done, for polio verification we need to go back to Area Clinic(My case Salmiya) and get stamp and doctor sign at vaccination department.
Then get back to Sabha Medical and submit the Medical Paper to Room No.4
Thats it. This will be communicated through Online to Visa Office.
Mean time to get Civil id number Corresponding to MOI Number (Visa Number 9 digits) call to 1889988 and follow instructions. You can get "Civil id number Corresponding to MOI Number" i thing under Civil ID query .
Go to Insurance office with Father Civil ID Copy, Visa Copy and Civil ID number. Pay 1 KD cash and 30 KD stamp to counter and your baby insurance is ready. Don't forget to get sign on the insurance receipt.
Along with your Visa Conversion Forms and photocopies take original insurance receipt and go to Visa Office to get stamping done. rest follow the same.
